Restoring Spouses or Domestic Partners to Health Coverage

Local 100 won an important victory, when, as a result of a PERB charge we filed together with ATU Locals 1056 &726, the MTA agreed to retroactively reinstate all spouses or domestic partners they had dropped from health coverage. This means that there will be no break in coverage and that any bills their spouse may have incurred will now be eligible for coverage.

To qualify for this retroactive reinstatement, you must submit by January 11, 2012 January 27, 2012 EITHER the required documentation OR two notarized affidavits, one from the member and the other from the spouse or domestic partner.

Anyone who submits the information after the deadline may have their spouse’s or partner’s coverage restored, but not retroactively.

For all members whose spouse or partner may have been dropped from coverage, we are posting the appropriate forms below for download.
